Post-Acute and Long-Term Care (PALTC) – Advanced Practitioner (AP)

The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is a full-time APRN or Physician Assistant providing independent, comprehensive medical care in PALTC settings.

  • Manage patient health through examinations, diagnoses, treatments, and preventive services.
  • Work autonomously within scope, making clinical decisions and collaborating with facility leadership and interdisciplinary teams.
  • Ensure compliance with regulations, maintain accurate documentation, and participate in quality improvement.
  • Oversee health and safety protocols, educate staff and patients, and promote best practices in hygiene and disease prevention.
  • Demonstrate strong communication, leadership, and technological skills, including EHR and telemedicine proficiency.
  • Work in-person at various long-term care facilities, with a focus on improving care quality, reducing hospitalizations, and controlling costs.

  • Qualifications include a relevant degree, 2+ years of experience (long-term care preferred), and active APRN/PA certification. Benefits include health, dental, vision, retirement, and paid time off.
