The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is a full-time APRN or Physician Assistant providing independent, comprehensive medical care in PALTC settings.
Manage patient health through examinations, diagnoses, treatments, and preventive services. Work autonomously within scope, making clinical decisions and collaborating with facility leadership and interdisciplinary teams. Ensure compliance with regulations, maintain accurate documentation, and participate in quality improvement. Oversee health and safety protocols, educate staff and patients, and promote best practices in hygiene and disease prevention. Demonstrate strong communication, leadership, and technological skills, including EHR and telemedicine proficiency. Work in-person at various long-term care facilities, with a focus on improving care quality, reducing hospitalizations, and controlling costs. Qualifications include a relevant degree, 2+ years of experience (long-term care preferred), and active APRN/PA certification. Benefits include health, dental, vision, retirement, and paid time off.
