Post-Acute and Long-Term Care (PALTC) – Advanced Practitioner (AP)

The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or Physician Assistant responsible for delivering independent, comprehensive medical care in PALTC settings.

They aim to

  • improve patient outcomes and care quality,
  • enhance the healthcare experience for patients and families,
  • reduce preventable hospitalizations,
  • and
    Key duties include conducting exams, diagnosing and treating illnesses, prescribing medications, and managing chronic conditions. They work collaboratively with facility staff and external providers, ensuring compliance with regulations, maintaining accurate documentation, and participating in quality improvement.

    The AP provides preventive care, educates staff and patients, and responds swiftly to urgent needs, often working independently. They must be proficient with EHR and telemedicine tools, possess strong communication skills, and meet licensure, certification, and experience requirements.
