Physician – Primary Care – Live Well Homecare

This position offers a $10,000 sign-on bonus per contract year for a board-certified primary care physician.

Key responsibilities include:

Coordinating care through a team-based model

Providing direct patient care

Ensuring patient rights are observed

Participating in training and meetings

Supporting clinic equipment and supply needs

Maintaining continuity of care across sites

Working collaboratively to meet organizational goals
