Physician Opportunities – Live Well Homecare

Seeking Board-Certified or Board-Eligible Family Medicine/Internal Medicine Physicians for immediate full-time roles in Colorado Springs, Pueblo, and Canon City, CO.

Provide compassionate, comprehensive care in post-acute and long-term care settings, collaborating with interdisciplinary teams.

Maintain proper documentation and meet quality, safety, and regulatory standards.

Offer Medical Directorship services with potential for additional earnings; minimal rotating weekend and holiday call (no night call).

Engage in care coordination, peer reviews, and quality improvement activities.

Travel between facilities as needed; reliable transportation required.

**Qualifications:**
– MD or DO with unrestricted license in Colorado
– Strong interpersonal skills and ability to work independently and in teams
– Experience in post-acute or skilled nursing settings preferred
– Familiarity with relevant EMR systems is a plus
