Role Overview: This position is for a Part-Time Clinician (Nurse Practitioner or Physician Assistant) responsible for conducting in-home health evaluations. The role involves building trusted relationships with patients, reviewing medical histories, performing physical assessments, and administering diagnostic tests as ordered by health plans. The clinician will use an iPad for evaluations, which typically last around 45 minutes, and may include virtual visits and local travel within licensed states.
Responsibilities:
- Provide comprehensive, personalized health assessments in patients’ homes
- Engage with patients to address health questions and review medication and medical histories
- Perform physical evaluations and diagnostic tests (e.g., spirometry, diabetic eye exams)
- Foster trust and connect patients to appropriate care based on their needs
- Work flexibly within licensed areas and support virtual visits when needed
- Deliver diagnostic and preventive services as indicated
Qualifications & Skills:
- Active, unrestricted licensure (NP or PA) in the coverage area
- Board certification (required)
- Preferred multi-state licenses
- Ability to work in diverse home environments, including those with pets or allergens
