Physician

Seeking a full-time Physician passionate about team collaboration and community-based care to support elderly and disabled individuals in maintaining independence.

  • Provide comprehensive medical and geriatric care as part of an interdisciplinary team, including care planning, monitoring, and leadership.
  • Supervise nurse practitioners or clinical nurse specialists at designated sites.
  • Deliver preventive, episodic, and health promotion services; perform physical, functional, and mental health assessments.
  • Evaluate, diagnose, and manage acute and chronic conditions, interpreting tests and adjusting treatments accordingly.
  • Educate members and families on health maintenance and illness management.
  • Travel locally using personal vehicle; work schedule is Monday to Friday, 8:00AM–4:30PM, with after-hours on-call managed remotely.

  • Requirements include a Doctorate in Medicine, MI licensure, DEA registration, Board Certification or eligibility in Family Medicine, Internal Medicine, or Geriatrics, and recent primary care experience with at-risk or elderly populations.
