Seeking a full-time Physician passionate about team collaboration and community-based care to support elderly and disabled individuals in maintaining independence.
Provide comprehensive medical and geriatric care as part of an interdisciplinary team, including care planning, monitoring, and leadership.
Supervise nurse practitioners or clinical nurse specialists at designated sites.
Deliver preventive, episodic, and health promotion services; perform physical, functional, and mental health assessments.
Evaluate, diagnose, and manage acute and chronic conditions, interpreting tests and adjusting treatments accordingly.
Educate members and families on health maintenance and illness management.
Travel locally using personal vehicle; work schedule is Monday to Friday, 8:00AM–4:30PM, with after-hours on-call managed remotely.
Requirements include a Doctorate in Medicine, MI licensure, DEA registration, Board Certification or eligibility in Family Medicine, Internal Medicine, or Geriatrics, and recent primary care experience with at-risk or elderly populations.
