Seeking a full-time Physician passionate about team collaboration and community-based care to support elderly and adults with disabilities.
Functions as part of an interdisciplinary team, developing care plans, monitoring health, and providing geriatric and medical expertise. Supervises nurse practitioners or clinical nurses at designated sites. Provides preventive, episodic, and chronic care, performs examinations, assessments, and manages treatment plans. Evaluates health status, orders tests, interprets results, and coordinates follow-up care. Educates members and families on health maintenance and illness management. Travel locally using personal vehicle; valid driver’s license required. Requirements include a Doctorate in Medicine, DEA registration, MI license, Board Certification or eligibility, with experience in primary care for at-risk or elderly populations.
