Physician – Live Well Homecare

This professional position involves providing complex, independent medical care to patients under clinical oversight from the Site Medical Director or Vice President of Clinical Affairs.

Requires graduation from an accredited medical school or certification from recognized boards.

Must hold and maintain active medical licensure in New Mexico, DEA, CSR, Medicare PIN, and Medicaid numbers.

Immunizations and testing (TB, Hepatitis B, Tdap, MMR, Varicella, influenza) are mandatory.

Additional requirements include BLS/CPR or ACLS certification within 30 days, with board certification in Family Medicine preferred.

Must pass criminal background checks if assigned to certain areas. The role involves adherence to funding contract terms and grants.
