We are hiring a full-time Physician to join a community-focused, mission-driven organization dedicated to helping elderly and adults with disabilities maintain independence.
Function as part of an interdisciplinary team, leading care planning and monitoring members’ health, including preventive, episodic, and chronic care.Provide supervision to nurse practitioners or clinical nurse specialists, perform physical, functional, and mental health evaluations, and interpret diagnostic tests.Treat acute and chronic conditions, manage medications, and develop follow-up plans based on member needs.Educate members and families on health maintenance and illness management.Requires MD/DO, DEA registration, NPI, CPR/AED, MI driver’s license, and board certification or eligibility in Family Medicine, Internal Medicine, or Geriatrics.Minimum one year of primary care experience with at-risk or elderly populations preferred.Work Monday to Friday, with after-hours on-call rotations, and travel as needed.Benefits include competitive pay, full or part-time schedules, generous paid time off, retirement contributions, flexible benefits, and more.
