Physician – Live Well Homecare

Seeking a full-time Physician passionate about team collaboration, community-based care, and supporting elderly and adults with disabilities to live independently.

Provide comprehensive care as part of an interdisciplinary team, including care planning, monitoring, and leadership.

Supervise nurse practitioners and clinical staff at designated sites.

Perform physical, functional, and mental health assessments; treat acute and chronic conditions; order tests; interpret results; and develop follow-up plans.

Educate members and families on health maintenance and illness management.

Travel locally using personal vehicle; flexibility required for after-hours and weekend on-call remotely.

Requirements include a Doctorate in Medicine, MI license, DEA registration, board certification or eligibility, and at least 1 year of primary care experience with elderly or at-risk populations. Geriatrics certification is a plus. Schedule is Monday to Friday, 8:00AM–4:30PM, with on-call rotation. Benefits include competitive pay, generous PTO, health insurance, retirement plan, and more.
