Physician

Seeking a full-time Physician passionate about team collaboration, community-based care, and supporting elderly and adults with disabilities to live independently.

  • Provide comprehensive care as part of an interdisciplinary team, including care planning, monitoring, and leadership.
  • Supervise nurse practitioners and clinical staff at designated sites.
  • Perform physical, functional, and mental health assessments; treat acute and chronic conditions; order tests; interpret results; and develop follow-up plans.
  • Educate members and families on health maintenance and illness management.
  • Travel locally using personal vehicle; flexibility required for after-hours and weekend on-call remotely.

  • Requirements include a Doctorate in Medicine, MI license, DEA registration, board certification or eligibility, and at least 1 year of primary care experience with elderly or at-risk populations. Geriatrics certification is a plus. Schedule is Monday to Friday, 8:00AM–4:30PM, with on-call rotation. Benefits include competitive pay, generous PTO, health insurance, retirement plan, and more.
