Seeking a full-time Physician passionate about team collaboration, community-based care, and serving the elderly and adults with disabilities.
Responsibilities include participating in interdisciplinary care planning, supervising nurse practitioners, providing preventive, episodic, and chronic care, performing evaluations, interpreting tests, and managing medications. The role involves education, documentation, and occasional local travel using a personal vehicle.
Requirements include a Doctorate in Medicine, MI licensure, DEA registration, Board Certification or eligibility in Family Medicine, Internal Medicine, or Geriatrics preferred, with at least 1 year of primary care experience with at-risk populations.
