Physician

Seeking a dedicated Physician to join a mission-driven organization focused on supporting elderly and adults with disabilities in maintaining independence.

Full-time role based at Mt. Pleasant or Houghton Lake sites.

Collaborate within an interdisciplinary team to develop and monitor personalized care plans.

Provide preventive, episodic, and chronic care, including physical and mental health assessments.

Supervise nurse practitioners and clinical staff at designated sites.

Manage acute and chronic conditions, adjust medications, and perform diagnostic evaluations.

Educate patients and families on health maintenance and illness management.

Travel locally as needed using personal vehicle.

Requirements: Doctorate in Medicine, MI license, DEA, NPI, CPR/AED, board certification or eligibility in Family Medicine, Internal Medicine, or Geriatrics, with at least 1 year of primary care experience with at-risk or elderly populations.

Schedule: Monday to Friday, 8:00 AM – 4:30 PM, with remote after-hours and weekend on-call rotation.
