Role: Licensed Physical Therapist specializing in in-home and senior living care, delivering personalized therapy to older adults in various settings, including private residences and senior communities.
Responsibilities:
- Provide one-on-one physical therapy, including evaluations, treatment planning, and evidence-based interventions.
- Support patient mobility, independence, and wellness through tailored therapy sessions.
- Document care accurately in electronic medical records, collaborate with interdisciplinary teams, and educate patients, caregivers, and staff.
- Incorporate telehealth and virtual care to enhance access and continuity.
Qualifications & Skills:
- Valid Physical Therapy license and degree from an accredited program.
- Proficiency in basic computer skills and EMR systems.
- Current CPR certification.
- Ability to meet physical demands, including lifting, repositioning patients, and performing therapeutic activities.
