Role: Physical Therapist providing personalized in-home and senior living care to older adults, focusing on improving mobility, independence, and quality of life through evidence-based treatments.
Responsibilities:
- Deliver one-on-one physical therapy sessions in private residences and senior communities
- Perform evaluations, develop individualized care plans, and deliver evidence-based treatments
- Support patient wellness, functional independence, and safety through therapy and education
- Document care effectively within the EMR system, adhering to regulations
- Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and fitness specialists
- Utilize telehealth/virtual care to extend access and flexibility when appropriate
Qualifications & Skills:
- Valid Physical Therapy license or eligibility in the practice state
- Degree from an accredited PT program
- Proficiency in basic computer and EMR systems
- Current CPR certification
Physical Requirements: Ability to perform moderate physical effort, assist with patient transfers, reposition patients, stand/walk for extended periods, climb stairs, and drive to patient locations. PPE may be required.
