Physical Therapist
Seeking a licensed Physical Therapist (PT) to provide patient assessments, develop treatment plans, and deliver therapy interventions based on physician directives within the home health setting. Responsibilities include evaluating mobility, strength, and function; documenting progress; modifying treatment plans; and collaborating with healthcare team members, patients, and caregivers. The role involves supervising personnel such as PT Assistants and Home Health Aides, completing assessments like OASIS where permitted, and ensuring compliance with policies, regulations, and documentation standards. Must possess a degree in Physical Therapy, valid state license, CPR certification, a driver’s license, and insurance.
