Role Overview: This position involves the evaluation and treatment of patients primarily in Rehab and LTAC hospitals, with responsibilities including conducting initial and discharge assessments, developing and implementing treatment plans, and documenting progress in the EMR. The role also includes educating patients, families, and staff, communicating patient progress within clinical teams, and coordinating with outside physicians and vendors for equipment needs.
Key Responsibilities:
- Perform assessments, set goals, and create treatment plans aligned with patient needs.
- Deliver timely and effective therapy to achieve functional goals.
- Identify educational needs and provide PRN therapy.
- Collaborate as part of an interdisciplinary team and serve as a mentor for interns.
- Participate in hospital and discipline-specific skills checks.
Qualifications & Skills:
- Bachelor’s Degree in Physical Therapy required; Doctoral Degree preferred.
- Minimum of 1 year experience in Rehab, SNF, LTACH, or Acute settings preferred.
- Current BLS certification and GA PT licensure or eligibility.
- Basic computer literacy.
