Role Overview: The Physical Therapist provides patient-centered care within a home health setting, managing approximately 25-35 patients across multiple counties. Responsibilities include developing and coordinating individualized treatment plans, supervising therapy staff, and ensuring positive patient outcomes through comprehensive rehabilitative interventions.
Key Duties:
- Assess, plan, and implement age-appropriate therapeutic care following physician orders.
- Coordinate multidisciplinary services throughout the patient’s episode of care, adjusting plans as needed.
- Supervise PTAs and Home Health Aides, ensuring quality care delivery.
- Educate patients and caregivers to promote independence and self-care.
- Communicate effectively with team members, physicians, patients, and families regarding patient progress and changes.
- Ensure documentation compliance with regulatory guidelines and participate in performance improvement activities.
- Adhere to safety, legal, and regulatory standards, including CMS, State, and Joint Commission requirements.
Qualifications & Skills:
- Graduation from an accredited physical therapy program.
- Current licensure as a Physical Therapist and BLS certification.
- Minimum of 1 year of full-time PT experience, preferably in home health.
- Strong assessment, rehabilitation, communication, and organizational skills.
- Proficiency with home health software, internet, and Microsoft Office tools.
- Valid driver’s license and private vehicle insurance.
