Overview
We are seeking a dedicated Physical Therapist to provide personalized, in-home rehabilitative care to patients recovering from surgeries, illnesses, or accidents. You will conduct comprehensive assessments, develop individualized treatment plans, and adjust care based on patient progress, working independently within an interdisciplinary team.
Responsibilities include:
- Practicing independently in the community.
- Assessing patient needs and creating treatment plans.
- Delivering tailored therapy in patients’ homes or care facilities.
- Monitoring patient progress and modifying care plans as needed.
Qualifications:
- Minimum one year of physical therapy experience or successful completion of a probationary period as a Rehab Intern.
- Licensure as a Physical Therapist in the state.
- Valid driver’s license outside Manhattan.
Compensation: USD $85,000 – $106,300 annually, with pay per visit rates depending on location and day of the week.
