Role: Licensed Physical Therapist providing personalized in-home, senior living, and virtual care to older adults.
Responsibilities: Conduct evaluations, develop individualized treatment plans, and deliver evidence-based therapy to improve mobility, independence, and quality of life. Document care using EMR, collaborate with interdisciplinary teams, educate patients and caregivers, and incorporate telehealth when appropriate.
Qualifications & Skills: Valid PT license, degree from an accredited program, proficiency in EMR and computers, current CPR certification, and ability to meet physical demands such as lifting, repositioning, and driving.
Key Duties: Support patient wellness, promote aging in place, and support safety through education. Flexibility in schedule creation, working in various settings, and utilizing virtual care options are emphasized.
