Role Overview:
Seeking a Physical Therapist to deliver patient-centered rehabilitation services in a supportive 1:1 care environment within a hybrid acute/inpatient setting. The role involves evaluating and developing treatment plans, educating patients and families, collaborating with healthcare teams, and delegating tasks to assistants and students. The therapist will document treatments in accordance with policies and provide assessments, reassessments, and consultations.
Qualifications & Skills:
- Licensed Physical Therapist with a Bachelor’s or Master’s Degree in Physical Therapy
- Strong communication, collaboration, and documentation skills
- Ability to establish effective treatment plans considering discharge needs and equipment
- Experience in evaluating and adjusting patient care
Key Responsibilities:
- Develop and implement individualized treatment plans
- Educate and collaborate with patients, families, and healthcare providers
- Delegate interventions appropriately based on competency levels
- Complete accurate and timely documentation
- Provide assessments, reassessments, and consultations for physical therapy services
