Role & Responsibilities:
The Home Health Physical Therapist provides comprehensive physical therapy services, including assessment, treatment planning, and therapeutic interventions within an interdisciplinary team. They are responsible for ensuring quality care, contributing to program development, and supporting quality improvement, problem-solving, and productivity efforts in a flexible environment.
Qualifications & Skills:
- Education: Must have a Doctorate of Physical Therapy (required after 2015)
- Experience: Relevant experience with specified duration and type preferred
- Licensure & Certification: Licensed Physical Therapist, Basic Life Support (BLS), Auto Liability Insurance, Reliable Transportation, Texas Driver’s License (all required)
- Additional Requirements: Valid Texas Driver’s License, auto liability insurance, dependable transportation, and ability to obtain certification/licenses within 90 days of hire
