Physical Therapist (Outpatient Rehab)
Seeking a compassionate, licensed Physical Therapist to provide high-quality, hands-on care in an outpatient setting. You will evaluate patients with musculoskeletal, neurologic, and post-surgical conditions and create evidence-based, individualized treatment plans to improve function, mobility, and independence.
- Perform patient assessments and develop tailored rehabilitation programs
- Deliver direct physical therapy treatments to support recovery goals
- Collaborate with a multidisciplinary care team
- Educate patients and families on conditions and home care strategies
- Document clinical findings and treatment plans in the EMR
Qualifications & Skills
- Active Physical Therapist license
- Strong clinical reasoning and patient-focused care
- Ability to apply evidence-based approaches to treatment planning
- Excellent communication and interpersonal skills
