Role: Physical Therapist
Responsibilities:
– Provide one-on-one physical therapy to older adults in private homes and senior living communities
– Conduct evaluations, develop personalized care plans, and deliver evidence-based treatments
– Promote patient wellness, mobility, and independence
– Document care within electronic medical records in compliance with regulations
– Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and fitness specialists
– Educate patients, caregivers, and staff on safety and aging in place
– Utilize telehealth and virtual care to enhance access and flexibility
Qualifications & Skills:
– Valid PT license or eligibility in the practice state
– Degree from an accredited PT program
– Proficiency in computer use and EMR systems
– Current CPR certification
Physical & Other Requirements:
– Ability to perform moderate physical effort, including lifting and repositioning patients
– Capable of driving to patient locations
– Willingness to wear PPE when necessary
