Role: Licensed Physical Therapist providing one-on-one geriatrics care in private residences and senior living communities.
Responsibilities: Conduct evaluations, develop individualized treatment plans, deliver evidence-based therapy, and support patient mobility, independence, and wellness. Document care accurately in EMR, collaborate with interdisciplinary teams, educate patients and caregivers, and incorporate telehealth when appropriate.
Qualifications & Skills:
- Valid Physical Therapy license or eligibility in practice state(s)
- Degree from an accredited program
- Proficiency in computer use and EMR systems
- Current CPR certification
- Ability to meet physical demands, including lifting, repositioning, and patient transfers
Key Duties: Provide personalized care across various settings, support aging in place, and utilize virtual care tools. Engage in a collaborative environment, manage treatment schedules independently, and contribute to patient outcomes that enhance mobility and quality of life.
