Physical Therapist
Provide patient-centered rehabilitation services in school, home, and facility settings. Evaluate functional mobility, assess patient needs, and develop evidence-based treatment plans to improve physical function and overall outcomes.
Key duties include:
- Conduct assessments and establish measurable goals for functional mobility and therapy progress
- Implement treatment plans using evidence-based physical therapy practices
- Document evaluations, interventions, and progress in accordance with regulatory and documentation requirements
- Collaborate with interdisciplinary teams to coordinate care and maximize outcomes
- Educate patients, families, and relevant staff on treatment plans and home/follow-up strategies
- Support diverse patient populations and uphold patient safety and compliance standards
Qualifications
- Current, active physical therapist licensure
- Strong clinical assessment, treatment planning, and patient education skills
- Ability to work collaboratively in interdisciplinary care environments and deliver compliant documentation
