Role Summary: The Pharmacist provides safe, effective, and cost-efficient medication therapy, developing and monitoring evidence-based care plans focused on patient outcomes and quality of life. They assess medications for indication, safety, effectiveness, and adherence, and monitor laboratory parameters and documentation.
Key Responsibilities:
- Deliver pharmaceutical care within designated patient care areas.
- Coordinate resources to ensure timely, efficient, and appropriate care.
- Interpret and clarify physicians’ medication orders, review patient medication profiles for drug interactions and contraindications.
- Review prescribing and formulary issues, suggest alternatives based on clinical parameters.
- Consult with medical staff on drug therapy problems and monitor lab results.
- Provide drug information to healthcare professionals and medication counseling for discharged patients.
- Participate in rounds, calculate dosages, verify orders, and prepare compounded or sterile medications.
- Report supply needs, supervise pharmacy staff, and ensure adherence to standards.
- Collaborate with healthcare teams to resolve pharmacy-related issues and assist in policy revisions.
Qualifications & Skills: Bachelor’s Degree in Pharmacy (required), Master’s preferred. Strong communication, collaboration, and clinical judgment skills are essential. Experience in medication management, compounding, and supervising pharmacy personnel is valued.
