Job Summary
The social worker provides complex social services to patients and their families from admission to discharge, collaborating with an interdisciplinary team.
Key responsibilities include assessing patient needs, developing care plans, and offering counseling and resource linkage.
Qualifications:
- High School Diploma, GED, or equivalent
- Degree in social work (dependent on licensure requirements)
Experience:
- Preferred: 1 year as Healthcare/Medical Social Worker
- Preferred: 1 year as Healthcare/Medical Case Manager
- Preferred: 2 years of counseling
Licenses and Certifications:
- Licensed Master Social Worker (LMSW) or Licensed Baccalaureate Social Worker (LBSW) with at least two years of social work experience must be current in Texas
- Basic Life Support (BLS) certification, required within ninety (90) days of start date
- SAFE Level III Training Course completion, required within six (6) months of start date
