Job Summary
The social worker is responsible for providing complex social services to patients and their families from admission to discharge, collaborating closely with the interdisciplinary team.
Key Duties
- Assess patients’ social needs and develop appropriate care plans
- Coordinate services and support for patients and families
- Advocate for patient needs within the healthcare environment
- Participate in interdisciplinary team meetings and treatment planning
- Ensure compliance with applicable regulations and standards
Qualifications
- High School Diploma, GED, or equivalent required
- Degree in social work (degree level dependent on licensure requirements)
Experience
- Preferred: 1 year as Healthcare/Medical Social Worker
- Preferred: 1 year as Healthcare/Medical Case Manager
- Preferred: 2 years of counseling experience
Licensure & Certifications
- Licensed Master Social Worker (LMSW) or Licensed Baccalaureate Social Worker (LBSW) with at least two years of social work experience, current in Texas
- Basic Life Support (BLS) certification within 90 days of start
- SAFE Level III Training Course completion within six months of start
