Position Overview
Seeking a Physical Therapist (PT) to join a home health care team, enabling patients to recover in their own homes.
Key Responsibilities:
- Conduct initial and ongoing assessments of mobility, strength, and function.
- Administer, evaluate, and interpret tests to set functional goals and outcomes.
- Provide written evaluations and treatment plans outlining needs, goals, and progress.
- Deliver physical therapy services following physician orders, maintaining documentation, and modifying treatment as needed.
- Plan discharge care, including patient and caregiver education on home exercises.
- Coordinate with healthcare team members and document according to policies, ensuring compliance and minimal supervision.
- Supervise clinical personnel (PTA, HHA) as applicable and complete assessments within state regulations.
Qualifications & Skills:
- Degree in Physical Therapy from an accredited program.
- Current, unrestricted state license as a Physical Therapist.
- Valid CPR certification and a current driver’s license.
