Role: LPN – Hospice
Responsibilities:
- Provide compassionate end-of-life nursing care in patients’ homes
- Administer medications and perform wound care under RN and physician supervision
- Monitor patient symptoms and report changes in condition
- Educate patients and families on comfort measures
- Coordinate with the interdisciplinary hospice team
- Support patients’ emotional and spiritual needs
Qualifications & Skills:
- Licensed Practical Nurse (LPN) license
- Strong communication and compassionate care skills
- Ability to work independently in patients’ homes
- Team-oriented with coordination skills
