Palliative Care Nurse Practitioner (NP)

We are seeking a dedicated full-time Palliative Care Nurse Practitioner (NP) to join our compassionate team in Rochelle, IL, and surrounding areas.

Manage a caseload of palliative patients, providing diagnosis, treatment, medication management, assessments, and consultations.

Travel to patients’ residences or facilities to deliver personalized, dignified care focusing on comfort and peace.

Develop and enhance our palliative care program while maintaining comprehensive hospice knowledge.

Collaborate with a multidisciplinary team committed to supporting patients and their families during end-of-life care.

Requirements include an active APRN license, RN license, and ability to obtain an NPI number. Preferred experience is 1-2 years in nursing, with a valid driver’s license and vehicle.
