Palliative Care Nurse Practitioner (NP)

We are seeking a dedicated full-time Palliative Care Nurse Practitioner (NP) to join our compassionate hospice team.

Manage a caseload of palliative patients, providing diagnosis, treatment, medication management, assessments, and consultations.

Travel to patients’ residences, including homes and nursing facilities, to deliver personalized care.

Develop and enhance the palliative care program within a supportive, team-oriented environment.

Maintain comprehensive knowledge in hospice and palliative care practices.

Collaborate with a multidisciplinary team to ensure patient comfort and dignity at the end of life.

Requirements include: a valid APRN and RN license, ability to obtain an NPI number, and a willingness to work on the road. Preferred experience includes 1-2 years of nursing.
