Provide comprehensive, patient-centered care for individuals with serious illnesses, focusing on symptom management and quality of life. Conduct assessments, develop personalized care plans, and collaborate with interdisciplinary teams for care coordination, discharge, and follow-up. Educate patients and families on treatment options, symptom control, and end-of-life decisions. Manage complex cases, utilize electronic health records, and participate in ongoing professional development. Qualifications: Master’s in Nursing with focus on Palliative Care, current Nurse Practitioner licensure, CDS and DEA licenses, with experience in critical care and hospice preferred. Strong communication skills and EHR proficiency are essential.
