Palliative Care Nurse Practitioner in Lafayette

We are seeking a compassionate Palliative Care Nurse Practitioner in Lafayette, LA, to provide holistic care to patients with serious illnesses.

Conduct diagnostic evaluations and develop individualized care plans.

Collaborate with interdisciplinary teams for care coordination, discharge planning, and follow-up.

Educate patients and families on treatment options, symptom management, and end-of-life decisions.

Manage complex critical care cases and utilize electronic health records for documentation.

Participate in ongoing professional development to enhance palliative care skills.

Qualifications include a Master’s in Nursing with palliative focus, current NP licensure, clinical experience in critical care, proficiency with EHR systems, and excellent communication skills. CDS and DEA licenses are required; hospice or palliative care experience is preferred.
