Role: Palliative Nurse Practitioner (Hospital-Based)
Responsibilities include:
- Respond promptly to inpatient palliative care referrals and perform comprehensive assessments addressing pain, symptoms, psychosocial, emotional, spiritual distress, and functional status.
- Develop and implement individualized care plans collaborating with an interdisciplinary team.
- Prescribe medications and treatments within scope, conduct ongoing reassessments, and facilitate discussions on prognosis, treatment options, code status, and advance directives.
- Support patients and families through complex decision-making and lead or support goals-of-care conversations.
- Coordinate care transitions to outpatient or hospice services, and serve as a clinical resource for hospital staff regarding palliative and hospice eligibility.
- Document assessments and care plans accurately in electronic medical records, ensuring compliance with policies and regulations.
- Participate in quality assurance, utilization review, and performance improvement activities.
Qualifications and Skills:
- Master’s or Doctoral degree in nursing with certification as a Nurse Practitioner (Adult or Family).
- Preferred hospice or palliative care nursing experience.
- Knowledge of palliative care frameworks and effective communication skills.
- Proficiency in computer use, problem-solving, critical thinking, and patient care planning.
- Maintains required continuing education, licensure, and certifications such as CPR.
- Valid driver’s license and willingness to travel, including evenings, weekends, and holidays.
