Outpatient Diabetes Care APP

We are hiring a compassionate and skilled Nurse Practitioner or Physician Assistant to join our outpatient diabetes clinic in Keene, NH.

Conduct patient visits, develop personalized care plans for diabetes management, glycemic control, and prevention.

Collaborate with endocrinologists, educators, dietitians, and nurses to provide holistic care.

Educate patients and families on disease management and lifestyle changes.

Order and interpret labs, adjust medications, and monitor outcomes.

Participate in quality improvement and program development.

Qualifications: NP or PA with NH licensure, relevant certification, and experience in endocrinology or primary care. Strong communication and clinical skills are essential.
