Outpatient Diabetes Care APP

We are seeking a compassionate and skilled Nurse Practitioner or Physician Assistant to join our outpatient diabetes clinic in Keene, NH.

Conduct patient visits, develop individualized care plans, and focus on disease prevention and glycemic control.

Collaborate with endocrinologists, educators, dietitians, and staff to provide holistic care.

Educate patients and families on diabetes management, lifestyle, and medications.

Order labs, adjust treatments, and monitor outcomes.

Participate in quality improvement and program growth initiatives.

Qualifications: License in NH, national certification, and experience in endocrinology or primary care with strong clinical and communication skills.
