On Call Visiting Nurse — Provide high-quality, compassionate, field-based nursing care across a Connecticut care network.
Key Responsibilities
- Respond to after-hours patient needs and conduct home visits.
- Assess patient conditions and nursing needs; monitor progress and changes.
- Administer prescribed treatments and deliver hands-on clinical care.
- Educate patients and families on care plans, medication, and self-management.
- Coordinate with interdisciplinary teams to support comprehensive care.
- Ensure accurate, timely clinical documentation and reporting.
- Support safe transitions of care and continuity between settings.
Qualifications & Skills
- Registered nurse licensure in the applicable state.
- Strong clinical assessment skills and safe medication/treatment administration.
- Excellent communication and patient/family education abilities.
- Ability to work independently in the field and collaborate with care teams.
