Role: Occupational Therapist responsible for providing patient-centered therapy services across various settings including acute care, TCU/LTC, swing bed, pediatrics, and outpatient adult care.
Key Responsibilities:
- Develops and implements individualized treatment plans based on patient assessments.
- Performs evaluations, monitors progress, and adjusts intervention strategies accordingly.
- Provides direct therapy using diverse functional activities tailored to patient needs.
- Coordinates care within interdisciplinary teams, communicating effectively with patients, families, and healthcare professionals.
- Documents evaluations, treatment plans, progress notes, and other reports accurately.
- Participates in program development, professional growth activities, and supervises clinical interns.
Qualifications & Skills:
- Active occupational therapy license in the practice state.
- Strong knowledge of age-specific patient needs from infants to geriatrics.
- Excellent communication, teamwork, and documentation skills.
- Ability to perform the essential functions, including visual acuity and effective hearing.
