Occupational Therapist – Home Health

Role Overview:

This position involves providing skilled occupational therapy services to patients through evaluation and treatment planning. The role includes establishing individualized treatment plans, educating patients and families, collaborating with healthcare teams, and delegating tasks to assistants and students.

Key Responsibilities:

Developing and implementing treatment plans considering discharge needs and required equipment.

Communicating and collaborating with patients, families, and healthcare providers, making referrals when necessary.

Delegating tasks based on competency levels and documenting services according to departmental policies.

Conducting assessments, reassessments, and consultations for occupational therapy services.

Qualifications & Skills:

Bachelor’s Degree in Occupational Therapy; Master’s preferred.
Valid Driver’s License and Occupational Therapist licensure.
No prior experience required; supervision experience not necessary.

Additional Requirements:

Ability to support alternative hours, including weekends.
Strong communication, collaboration, and organizational skills.
