Role Overview: The Occupational Therapist provides age-appropriate rehabilitative care in patients’ homes, addressing physical, emotional, developmental, psychosocial, and educational needs. They collaborate with multidisciplinary teams to develop and revise individualized care plans, deliver therapeutic interventions, and ensure high-quality, continuous patient care.
Key Responsibilities:
Assess and manage patients’ rehabilitative needs, scheduling visits per medical orders and adjusting as necessary. Perform routine occupational therapy procedures and meet productivity standards. Coordinate care with team members and report changes to physicians. Educate patients and caregivers to foster independence and support recovery. Participate in staff education, training, and continuing education to stay current in clinical practices. Accurately document interventions, communicate with the care team, and adhere to regulatory guidelines.
Qualifications & Skills:
Graduate of an accredited Occupational Therapy program. Licenses: Occupational Therapist license, BLS, BLS Instructor, and Provisional certifications as required. Minimum of 1 year of full-time occupational therapy experience; home health experience preferred. Strong assessment, rehabilitative, organizational, and communication skills. Proficiency with electronic medical records, home health operations, and regulatory standards. Valid driver’s license and insurance for home visits.
