Nurse Practitioner – Spanish Required

Seeking a Nurse Practitioner to deliver equitable, value-based primary care for Medicare adults in outpatient settings.

Provide independent, comprehensive care including routine, acute, and wellness visits, focusing on building patient relationships.

Coordinate care with specialists, test facilities, and agencies while leading population health initiatives.

Educate patients, assist with triage, and participate in outreach and promotional activities.

Conduct home visits as needed and collaborate with a multidisciplinary team including physicians, nurses, and support staff.

Qualifications: Master’s or DNP degree, national certification (FNP, AGNP, ANP, GNP), active licenses, and US work authorization. Preferred experience in primary care, understanding of coding and health assessments, and bilingual skills are a plus.
