Nurse Practitioner – Spanish Required

Seeking a Nurse Practitioner to deliver equitable, value-based primary care to Medicare adults in outpatient settings.

Responsibilities include conducting office visits, wellness exams, care coordination, health education, and participating in population health initiatives. The role involves collaboration with a multidisciplinary team and may include home visits and outreach activities.

Requirements:

Master’s or DNP degree and national certification in family, adult, or gerontological primary care

Active NP license, DEA registration, and US work authorization

Preferred experience in primary care, HCC/ICD-10 documentation, and bilingual skills
