Nurse Practitioner – Spanish Required

We are seeking a Nurse Practitioner to deliver equitable, value-based primary care for adult Medicare patients in outpatient settings.

Provide comprehensive primary care, conduct routine and acute visits, and lead annual wellness assessments.

Collaborate with a multidisciplinary team, coordinate care with specialists, and support population health initiatives.

Educate patients, assist with triage, and participate in outreach and promotional activities.

Conduct home visits as needed and contribute to team efforts.

The role reports to the Center Medical Director and requires an MSN, DNP, or NP certification, along with active licensing and DEA registration. Preferred experience includes primary care, population health, and bilingual skills.
