We seek an experienced APRN in pulmonary care to provide patient-centered services in skilled nursing and long-term rehab settings, working in person 2 days/week, including some weekends and on-call.
Responsibilities include managing complex respiratory conditions, performing assessments, managing ventilator therapies, tracheostomy care, prescribing medications, and coordinating with interdisciplinary teams. The role involves documentation, patient education, and initiatives to reduce hospital readmissions, ensuring compliance with regulations.
Qualifications include an active APRN license, Master’s degree, 6+ months in long-term care, 3+ years nursing experience, BLS/ACLS, EMR proficiency, and knowledge of post-acute care. Ideal candidates are organized, compassionate, and independent.
Compensation ranges from $60,000-$90,000/year with benefits like paid time off, malpractice insurance, ongoing education, and AI-assisted documentation. This in-person role is based in Forest City, PA.
