We are seeking an experienced APRN in pulmonary care to provide patient-centered services in skilled nursing and long-term rehabilitation settings. This part-time, in-person role requires availability for 2 days per week, including weekends, with occasional on-call coverage.
Responsibilities include managing complex respiratory conditions, performing evaluations, managing ventilator and trach care, prescribing medications, and coordinating care with interdisciplinary teams. The role emphasizes reducing hospital readmissions and ensuring compliance with healthcare regulations.
Qualifications include an active APRN license, Master’s degree, 3+ years of nursing experience, and experience in long-term or post-acute care. Strong clinical skills, EMR proficiency, and excellent communication are essential.
Compensation ranges from $60,000 to $90,000 annually, with benefits like paid time off, malpractice insurance, ongoing education, and AI-assisted documentation. This position requires in-person work at Forest City, PA.
