We are a growing physician practice seeking a Medical Provider to deliver high-quality care across multiple facilities, including Skilled Nursing, Assisted Living, and Independent Living settings.
- Provide medical care, maintain documentation, and participate in medical staff duties, including meetings and peer reviews.
- Travel between multiple locations to evaluate and coordinate patient care, ensuring compliance with regulations.
- Contribute to quality assurance, utilization management, and operational improvements.
- Support strategic initiatives and foster positive relationships with staff, providers, and the community.
- Participate in committees to optimize relationships with payors and peers.
- Share weekend and holiday call duties as per agreement.

The position requires relocation before starting in Grand Junction, CO. Salary ranges from $110,000 to $135,000 annually, with full-time hours and supplemental pay options.
