Seeking a Nurse Practitioner or Physician Assistant for an interventional pain management clinic with multiple locations, including physical therapy, satellite, and surgery centers.

Responsibilities:
Conduct assessments, diagnoses, and develop care plans
Order, interpret, and follow up on diagnostic tests
Document patient care thoroughly
Provide care per standards and collaborate with the team
Monitor and refer patients as needed
Requirements:
Current applicable state license and national certification
DEA license or ability to obtain
Basic Life Support certification
Job is full-time with a salary range of $125,000 to $150,000 per year.
The role requires relocating to Sutherlin, OR before starting. In-person work only.
