We are seeking a Part-Time Nurse Practitioner (NP) to support Skilled Nursing and Post-Acute facilities in Georgetown, SC.
Schedule & Environment:
- Part-Time, 3 days/week (Monday–Friday), 8:00 AM – 4:00 PM
- On-site at healthcare facilities
- No nights, weekends, or on-call duties
Responsibilities include evaluating and managing admissions and follow-ups, providing chronic disease management and preventive care, collaborating with care teams, maintaining clinical documentation, and serving as an on-site clinical representative.
Qualifications:
- Active South Carolina NP license and Board Certification
- Strong clinical judgment and documentation skills
- Experience in post-acute or outpatient settings preferred
- Interest in geriatrics and continuity of care valued
